After you install FreePPP, you'll see this icon 
in your menu bar in the upper right hand corner.
When you click on it, you'll see the options "Open FreePPP Connection",
"Close FreePPP Connection", and "Open FreePPP Setup". The screen you see
above is what you have after you select the Setup option.
Click on the little triangle in the lower left hand corner to see the configuration screens.
